Medikinet and ADHD

Medikinet is an ADHD medication containing methylphenidate, available in the UK as both an immediate-release tablet and an extended-release capsule (Medikinet XL). Medikinet XL uses a bead system that delivers half the dose immediately and half after a delay, providing approximately 8 hours of coverage from a single morning dose taken with breakfast. ADHDose models both versions, tracking your levels in real time.

How Medikinet XL works through the day

Medikinet XL capsules contain two types of beads. Approximately 50% are immediate-release beads that dissolve quickly after swallowing, providing early morning coverage. The other 50% are coated with a delayed-release layer that breaks down later, typically 3 to 4 hours after taking the capsule, providing a second wave of medication.

This two-phase design creates a profile with two peaks: one within the first one to two hours, and a second around four hours after dosing. The overall duration is approximately 8 hours, which is shorter than Concerta XL (approximately 12 hours) but covers the core of the working or school day.

Why Medikinet XL must be taken with food

This is one of the most important practical differences between Medikinet XL and other extended-release methylphenidate formulations. The delayed-release beads in Medikinet XL rely on food in the stomach to trigger the second phase of release properly. Taking it on an empty stomach can cause the second phase to release too early, reducing both the duration and consistency of coverage.

Your prescriber will advise taking Medikinet XL with breakfast or immediately after eating. If you skip breakfast, the medication may not work as intended that day.

Medikinet XL capsules can be opened

Unlike Concerta XL tablets which must be swallowed whole, Medikinet XL capsules can be opened and the beads sprinkled onto a small amount of soft food such as yoghurt or apple sauce. The beads must then be swallowed without chewing or crushing. This is useful for people who find capsules difficult to swallow.

Medikinet IR (immediate-release)

Medikinet is also available as immediate-release tablets, which work identically to Ritalin: onset within 20 to 30 minutes, lasting 3 to 4 hours, taken two or three times daily. Your prescriber may use the IR version during early titration to establish the right dose before switching to XL.

Medikinet XL vs Concerta XL

Both are extended-release methylphenidate, but the delivery mechanisms and durations differ. Concerta XL uses an osmotic pump for approximately 12 hours of ascending-rate delivery. Medikinet XL uses a 50/50 bead system for approximately 8 hours with two distinct peaks.

The choice between them often comes down to how many hours of coverage you need. Medikinet XL suits people who want coverage through the working day but prefer the medication to have cleared by the evening. Concerta XL suits people who need longer coverage, including the commute home and early evening.
